Luxe Membership Enrollment
Your Luxe Membership is here and ready to unlock special member pricing and savings on your treatments and services. You’ll also have the option to open a Luxe Bucks Account and continue setting money aside toward your favorite treatments and services.
For your security, no payment or credit card information is collected through this form. This is an enrollment form only. Once submitted, a member of our team will contact you shortly to finalize your membership and collect payment information securely.
Luxe Membership Terms
By re-enrolling in the Luxe Membership, you acknowledge the following:
$99/month for a 12-month membership term. First month’s dues are collected at enrollment, followed by 11 monthly auto-drafts.
A minimum 3-month commitment is required.
Cancellation during the initial 3-month commitment is subject to a $125 early cancellation fee.
Membership dues are non-refundable and non-transferable and cannot be applied to the cost of treatments, products, and services.
Luxe pricing and benefits are available only while the membership is active and dues are current.
Luxe Bucks is an optional monthly auto-draft and can be used towards the cost of treatments, products, and services.
Manufacturer rewards are separate from Luxe Membership benefits. Allē and Galderma rewards may apply where eligible; Evolus and Revance rewards/perks may not be combined with Luxe Membership pricing.
Management reserves the right to modify membership pricing, benefits, eligible services, and program terms as needed.
